Physical exams:

• are required for all student participants in order to participate in practices and tryouts
• are valid for 2 years from the exam date
• MUST BE ON THE CURRENT OSAA FORM ONLY (current version: April 2023)
• need to be uploaded to your student's Arbiter registration or turned into the Athletics office
• are the responsibility of the family
• need to be signed by the student and the parent/guardian on Part 1 of the form
• and signed by the physician clearing the student on Part 2 of the form.

Free sports physicals can be scheduled through the Franklin Student Health Center (usually within a few days during the school year) at 503.988.3370. You can also schedule a physical through a clinic or your personal doctor’s office.
